This note is circulated and has the following: folding and wear (also has some heavier creases); overall staining and darker areas of spot/line/section stains (also has smudges, dark and dirty areas, grease stains, areas of discoloration); edge and corner wear (including corners rounded and/or split/frayed - edges rough in areas with areas of edge fraying); edge indents/nicks (some tears exist and one is larger); surface wear and fraying (areas of paper loss exist); no holes that I could see.
